During the program year, we ask that you complete the Physician Certification Form and Health Risk Assessment. Once these forms are complete, you can simply upload your Physician Certification Form which acknowledges your Health Risk Assessment completion to your Healthworks portal, and you will receive credit. Remember to take your completed Health Risk Assessment form to your physician so he/she can acknowledge the completion on your Physician Certification Form. You do not need to upload the Health Risk Assessment, only the completed and signed Physician Certification Form. New participants in the plan must submit forms no later than 60 days following initial plan eligibility. You can receive 1 physical per calendar year. Meaning, if you received your last physical in December 2026, you are eligible go back for another physical anytime in 2027. You do not have to wait until December 2027
*New Hires: Annual physical exams conducted within the past 12 months are acceptable for your “initial” submission.

As newly enrolled members of CTU follow the instructions below to submit your Wellness Form:
To qualify for the wellness incentive, eligible members must submit a Physician Certification Form which includes the following activities.
- Physician Certification Form Includes Verification of the Following (actual results, diagnoses, and/other details of testing or assessment are not to be included):
- Cholesterol Screening
- Glucose Screening
- Blood Pressure Screening
- Body Mass Index (BMI)
- CMSD Health Risk Assessment (Physician acknowledges completion on the Physician Certification Form)

Create your Healthworks account:

- Click myhealthworksportal.com to enroll
- If you haven’t already, click “Sign up now” and create account!
*Input your name as it appears on your drivers license - A verification code will be sent to the email address you provide
- After your account has been verified, you will be prompted to enter your company’s access code: 69BTP
Please note: You are encouraged to keep forms for your records.
How to submit your Physician Certification Form:
- Login to your Healthworks portal
- On your Wellness Checklist, click “Annual Physical Verification”
- Use “Click Here to Upload a Document for Review” to upload your form
- A “Document Uploaded Successfully” box appears once uploaded
