WELLNESS INCENTIVE

Take Charge of Your Health & Well-being

Here at Cleveland Metropolitan School District, your health and wellness is one of our top priorities. Our goal is to provide the proper tools and resources to help guide you to be the best and healthiest version of you. NEW for this year, we are excited to announce your wellness portal, powered by PeopleOne Health. Your new wellness portal is now the hub for your annual form upload to earn your wellness incentive towards your healthcare premiums. In addition to the upload feature, you will have access to on-demand health and wellness features to help you focus on your health and well-being goals.

During the program year, we ask that you complete the Physician Certification Form and Health Risk Assessment. Once these forms are complete, you can simply upload your Physician Certification Form which acknowledges your Health Risk Assessment completion to your PeopleOne Health portal, and you will receive credit. Remember to take your completed Health Risk Assessment form to your physician so he/she can acknowledge the completion on your Physician Certification Form. You do not need to upload the Health Risk Assessment, only the completed and signed Physician Certification Form. New participants in the plan must submit forms no later than 60 days following initial plan eligibility.

Who is Eligible?

The Cleveland Metropolitan School District has established a wellness incentive for eligible participants who complete the below list of screenings and Health Risk Assessment. To qualify, the member must have submitted a Physician Certification Form which includes the following activities.

  1. Physician Certification Form Includes Verification of the Following (actual results, diagnoses, and/other details of testing or assessment are not to be included):
    • Cholesterol Screening
    • Glucose Screening
    • Blood Pressure ScreeningBody Mass Index (BMI)
  2. CMSD Health Risk Assessment (Physician acknowledges completion on the Physician Certification Form)

The forms are available on your PeopleOne Health wellness portal.

Create your PeopleOne Health account:

  1. Click portal.peopleonehealth.com/enrollment/CMSD to enroll
  2. Create a password
  3. All set!

How to submit your Physician Certification Form:

  1. Login to your PeopleOne Health portal
  2. Click ‘Programs’ on the navigation menu
  3. Scroll to ‘Physician Certification and Health Risk Assessment’ program step and click to dropdown
  4. Click blue bar ‘Physician Certification and Health Risk Assessment’, download and print form
  5. Download ‘CMSD Physician Certification Form’ and Print
  6. Download ‘CMSD Health Risk Assessment Form’ and Print
  7. Provide both forms to your physician for signature and upon form completions, click ‘Choose’ and locate your complete form, click ‘Add File’
  8. This step will be marked with a gold checkmark upon completion.
  9. Congratulations, you’re all set!

Please note: You are encouraged to keep forms for your records.

How to submit a Wellness Incentive Appeal

  • Dowload the Wellness Incentive Appeal Request Form from the CMSD Employee Benefits webpage
  • Submit the form and supporting documentation to [email protected]

Appeals must be submitted within 60 days of when you knew or should have known of the event for which the relief is requested.

We are interested in your total well-being. That is why we offer an Employee Assistance Program. This program provides a counseling service that helps you manage problems before they adversely affect your personal life, health and job performance.

Create a MyLifeExpert account through the Ease@Work website by taking these steps:

  • Visit mylifeexpert.com/login
  • Select ‘Sign Up’ in the top-right corner
  • Enter Company Code: cmsdist
  • Provide your District Email Address
  • Create your username and follow any final prompts for finalizing your account

Life can be complicated. There are times when everyone needs a little help or advice. The confidential Employee Assistance Program (EAP) can help you with things like stress, anxiety, depression, chemical dependency, relationship issues, legal issues, parenting questions, financial counseling, daily living services, personal wellness and dependent care resources. If you’d like help handling life’s demands, contact the EAP for extra support. Assistance is only a click or phone call away.

You have three face-to-face sessions with a behavioral counselor available to you and your house-hold members. Call us to request a referral.

ACHIEVE WORK/LIFE BALANCE

If you’d like help handling life’s demands, call the EAP for extra support. They can refer to a service in your community.
